REQUEST FOR PROPOSAL (RFP)
FOR ENGINEERING DESIGN SERVICES

The scope of work
includes all phases of design from design schematic through construction
administration and project close out. The designer will prepare complete plans
and construction bid documents in accordance with Chapter 7, Section 38A1/2 –
Section 380 of the Massachusetts General Laws and will assist the town in the
public bidding of the project with the goal of having the construction contract
let out no later than Nov. 1, 2010.
Interested parties should
submit their proposal with a statement of their qualifications and experiences
via email marked “Design Services” Town of Egremont Photovoltaic System and
Thermal Upgrades, to Egremont Green Committee at the following addresses: juliettehaas@yahoo.com, mwexler43@gmail.com, robngold@hotmail.com, spbach@aol.com. chuckog@hotmail.com, jverones@hughes.net.
Proposals must include the following:
a. Detailed statement of the firms’ qualifications and experience
with projects of this type.
b. Specific names, resumes, and responsibilities of individuals to
be assigned to the project and their specific involvement.
c. Proposed Scope of Work to include all costs of compiling
necessary information and data to generate plans and specifications,
preliminary and final estimating and design, project review and consultation
with the owner, coordination of preparation of bid documents, drawings and
specifications, local plan review attendance of a pre-bid meeting, and
construction site inspections, construction administration services necessary
to implement the project including review of submittals shop drawings, schedule
of values, change order and payment applications and issuance of certificate of
completion.
d. Submit a separate fee proposal for each aspect of the project
and segment of the proposed Scope of Services and a copy for the firm's per
diem rates. All proposals must identify costs for each work item. Any
sub-consultants should be identified and the responsibility for completion of
work and coordination between disciplines should be detailed. No percentage of
cost proposals will be accepted. Total Design Fees Not To
Exceed $14,000.
e. A proposed time schedule for completion of the total project
broken down by phases.
f. Prior to the issuance of a contract, the proposer will be
required to submit an insurance certificate in amounts and types required by DOH
pursuant to Bulletin #94-003 as well as all other certificates as are required
under M.G.L. c.7 section 38H(e).
g. A copy of the firms’ Affirmative Action Policy or Plan, and if
applicable, it’s State Department of Economic Development Small/Minority/Female
Business Certification.
